Landlord Kit

Instructions for registering with DHAP:

Step 1:

Contact DHAP at (866) 202-3487 to determine the Phase of your tenant(s). There are two (2) forms that are specific to the Phase of the tenant.

Step 2:

Please download and complete the following docs:

• Direct Deposit
• Disaster Rent Subsidy Contact (select the form according to the phase of each tenant.

	>	Disaster Rent Subsidy Contract
	>	Disaster Rent Subsidy Contract for Phase 2 Families

• Lease Addendum (select the from according to the Phase of each tenant)

	>	Lease Addendum
	>	Lease Addendum for Phase 2 Families

• Lead Based Paint Disclosure
• Request for Tenancy Approval
• W-9
• Vacancy Listing Form (Please complete so that we can gather/store info on your unit)

Step 3:

Please also provide a copy of the following:

• A voided check (with proper routing info) for Direct Deposit
• Proof of Ownership for each property registered
• Current Lease agreement with each DHAP/CLC/FEMA tenant
• Rent Roll (listing of all tenants currently receiving rental assistance through DHAP/CLC/FEMA

NOTE: If you are registering more than one tenant, the only forms that must be submitted for each are:

• DRSC (For appropiate Phase)

	>	Disaster Rent Subsidy Contract
	>	Disaster Rent Subsidy Contract for Phase 2 Families

• Lease Addendum (For appropiate Phase)

	>	Lease Addendum
	>	Lease Addendum for Phase 2 Families

• Lead Based Paint Disclosure
• Request for Tenancy Approval
